Resolution of Safety and Health Issues Flow Chart

Safety and health issues identified in the workplace must be addressed in accordance with the following procedure. Staff, students and visitors who have identified an issue shall:

  • Report the issue to their immediate supervisor, manager, lecturer or tutor and attempt to resolve the matter with them.
  • Seek advice and assistance where necessary from their elected safety and health representative or Occupational Safety and Health.
  • Request the safety and health representative or Occupational Safety and Health assist in resolving the issue through senior management where satisfactory resolution cannot be achieve a through line management.

Where all avenues to resolve the issue have been exhausted internally in accordance with the provisions of this procedure, the matter shall be referred to WorkSafe, WA.

Upon resolution of the issue, the safety and health representative and the person reporting the issue shall be provided with feedback.
